Notice Regarding IRS Form 1095-C Availability for Employees

In accordance with IRS regulations, Aspen Technical Staffing will provide Form 1095-C, which contains important information about the health coverage offered to you during the tax year, upon request.

Requesting Form 1095-C Electronically

Employees who wish to receive their Form 1095-C in electronic format must submit a written email request then provide affirmative consent in compliance with IRS requirements (26 CFR § 301.6056-2). Paper copies will not be issued unless specifically requested.

To request your electronic Form 1095-C and provide the required consent, please contact:

Once we receive your request via email, we will send the electronic consent form. Upon confirmation of your consent, you will be granted secure access to your Form 1095-C electronically.

Important Notes:

• You must provide consent before the form can be delivered electronically.

• If you do not request the form or do not provide consent, no electronic copy will be issued.

• You may withdraw your consent at any time by notifying Aspen Technical Staffing.
